Chronic Disease Learning Pathway

Expression of Interest

We are launching an Expression of Interest for the 2023 rollout of the Chronic Disease Project Workshops - Chronic Conditions of the Foot, Chronic Conditions and Nutrition, and Chronic Disease Care Planning.

Benchmarque Group has been engaged by the Department of Health for the delivery of the Chronic Disease Support Project, in particular, the delivery of the Chronic Disease Learning Pathway.

The activity utilises the Chronic Disease Learning Pathway to engage Aboriginal and/or Torres Strait Islander Health Workers and Practitioners and other health professionals to assist Aboriginal and/or Torres Strait Islander peoples who live with chronic disease to achieve better health outcomes.

The pathway begins with all students undertaking study in the Foundations of Chronic Disease program, delivered online and for free. The student then selects their Chronic Disease journey and expands on their learning to include nationally accredited programs.

All programs are fully funded by the Department of Health, which means there is no cost for participating organisations or individuals.

Following participation and successful completion of the programs, students will be able to:

  • Assist patients with chronic diseases in managing their condition/s better
  • Support patients to recognise changes in their health and understand the need to seek timely treatment
  • Provide client-focused services.

If you previously applied for this activity last year, many thanks for your support. 2021-2022 was challenging to implement in the COVID-19 environment, and we encourage you to resubmit a new application for program delivery again. We also encourage all new clients and stakeholders to apply to the rollout of this project.

Applications close on 30 November 2022. They will be prioritised and processed according to the order of submission. 

Ongoing applications will be accepted on an ad-hoc basis through 2023.
