Application Process
Applications for the Graduate Certificate in Advising on Chronic Disease Self Management will be accepted in writing only. Please open and print the application form, complete and return to The Benchmarque Group on:

On application you have the choice of paying the entire course fee on registration and taking advantage of a full payment discount. Alternatively for the 2012 Program you are able take advantage of our instalment plan. If you register prior to January 1st 2012 you can apply to pay for your course an pay in 10 affordable instalments deducted on the 15th of every month.
All applications will be date stamped and processed based on date of receipt.
